Schedule your initial consultation at least 4 to 6 weeks prior to your departure to ensure all immunizations are effective. Call 713.798.7700 to make an appointment.
If you prefer, you may download, print and complete the paper form (PDF, 202K, 3 pages). (Requires the free Adobe Acrobat Reader on your computer.) Submit the
completed printed questionnaire by fax to 713.798.2752 or mail it to 3701 Kirby,
Suite 100, Houston, TX 77089. The Pre-Travel Questionnaire must be received at least
2 business days prior to your appointment.
Include copies of all previous immunization records available to you. If necessary,
contact the appropriate persons to obtain these records. This information is necessary
for Baylor Travel Medicine nurses to review prior to your arrival.
If you have an "International Certificate of Vaccination" (yellow booklet), bring
it with you to your appointment.
NOTE: Failure to provide us with your complete information as requested may result
in rescheduling your appointment, or additional visits and expense.
During Your Consultation
Allow one hour for your first visit (add additional time for couples, families,
etc.). During your visit we will:
Review your itinerary, medical history, medications and previous vaccinations.
Counsel you on required and recommended vaccines for your destination and administer
those you’ve agreed to that day. You may need to return for any vaccines given
in a series.
Provide guidelines on insect bite prevention, food and water precautions and any
other destination-specific issues.
Provide prescriptions, when appropriate, for self-treatment of travelers' diarrhea,
malaria prevention and altitude sickness.
Provide a detailed report regarding the countries you are visiting.
Our Travel Medicine nurse will conduct the counseling session.
Dr. Jane Corboy, associate professor of Baylor
College of Medicine's Family and Community Medicine Department, may review your
Pre-Travel Questionnaire and provide recommendations regarding your appointment.
After Your Consultation
Payment in full is required at the time of service. Please refer to the
Charges and Payments section
for information on payment, fees and insurance.
Please keep any follow-up appointments so you acquire the maximum protection the
vaccines offer. Altering or delaying recommended vaccination schedules could adversely
affect your protection and put you at risk for illness.
