New Patients


Please call the office before your first appointment to have your prior medical records transferred. You will need to sign a release before your previous doctor can send your records. Medical release form

The day of your first appointment, arrive 30 minutes before your appointment. Bring your current insurance card and driver's license or picture identification. Medicaid/Medicare patients need to have a valid card and letter, as well as secondary insurance information. Be prepared to complete paperwork. For your convenience, you can print the new patient registration form and have it completed, bringing it with you to your first appointment.

Open Access Scheduling


Baylor Family Medicine is pleased to announce that we have adopted Open Access Scheduling. Open Access will allow us to schedule most appointment requests within 24 hours.

Open Access is a new patient scheduling concept that emphasizes doing "today's work today," rather than scheduling appointments weeks or months in advance. Physician offices that have adopted Open Access scheduling now treat 50 percent of patients on the same day they call to make an appointment. The remaining patients are seen within five business days, usually by choice. More information...
